Understanding what records to bring — and what to do if you don't have them — before your USCIS Form I-693 exam.
Acceptable Vaccine Documentation
We accept a wide range of vaccination records. Bring whatever you have — we will work with it.
US Immunization Records
CDC yellow vaccination cards, state immunization registry printouts, and records from US physicians or clinics are all accepted.
Foreign Vaccination Records
We accept immunization booklets and records from any country. Foreign-language records are acceptable — formal translation is not always required.
School Records
School immunization records are valid documentation, especially for children. Bring any school-issued vaccination forms you have.
Military Records
Military immunization records are accepted and often very complete. Veterans and active-duty applicants should bring their immunization history.

Translation Requirements
Formal translation of vaccination records is not always required. Our staff is experienced with international immunization documentation and can often work directly with foreign-language records.
Acceptable Formats
Foreign immunization booklets, national vaccination cards, and records from foreign physicians or clinics are all acceptable. Bring originals when possible.
Incomplete Foreign Records
If your foreign records are incomplete or unclear, we will review your history and administer any required catch-up vaccines at our office. This is a common situation and will not delay your exam.
Tip: Bring all records you have — even partial documentation helps us minimize the vaccines you may need to receive.
Prior Lab Results
Prior TB Tests
If you have had a prior TB skin test (TST) or IGRA blood test, bring those results. Prior positive TB tests require documentation of treatment. Our civil surgeon will review your history and determine the appropriate testing approach.
IGRA Results
QuantiFERON-TB Gold or T-SPOT results are relevant to your immigration exam. Bring any prior IGRA results, especially if you have had a positive result or received TB treatment.
Prior Syphilis Tests
If you have had prior syphilis testing, bring those results. Prior positive syphilis tests require documentation of treatment. Our civil surgeon will review and document your history appropriately for USCIS.
Organizing Your Records Before Your Appointment
- Gather all vaccination records from every provider you have seen
- Request records from your state immunization registry (available in most states)
- Contact your pediatrician or primary care physician for historical records
- Check school files for immunization records if you attended US schools
- If you have foreign records, bring originals — we can work with them
- Place all documents in a folder or envelope to keep them organized
- Write down any vaccines you recall receiving even if you lack documentation
What If Records Are Missing?
Missing vaccination records are one of the most common situations we encounter. Here is how we handle it:
Incomplete Vaccination History
We will review what you have and administer any required catch-up vaccines at our office. We stock all USCIS-required vaccines. Missing records will not prevent you from completing your exam.
No Records at All
If you have no vaccination records, we will start fresh. We will administer all required vaccines and document your complete vaccination history for USCIS. This is a common situation — do not let it stop you from scheduling.
Catch-Up Vaccines
USCIS requires specific vaccines for immigration applicants. If you are missing any, we administer them right at our office. No outside pharmacy or clinic visit is needed.
Titers Testing
In some cases, a blood titer test can demonstrate immunity even without vaccination records. Our civil surgeon will advise if titers testing is appropriate for your situation.
