What to Bring to Your Immigration Medical Exam
Arrive prepared and avoid delays. Here is everything you need to bring to your USCIS Form I-693 immigration medical exam at our Costa Mesa clinic.
Required Documents Checklist
Bring all of the following to your appointment. Missing items may delay your exam or require a return visit.
Government-Issued Photo ID
- Passport (strongly preferred — required for most applicants)
- Foreign national ID card if passport is unavailable
- Driver's license as secondary ID only
- Permanent Resident Card (if applicable)
USCIS Paperwork
- I-485 receipt notice (if available)
- Any USCIS correspondence related to your case
- Attorney referral letter (if applicable)
- Prior I-693 forms (if you have had a previous exam)
Vaccination Records
- All prior immunization history — US or foreign
- School vaccination records
- Military vaccination records
- Any written vaccine documentation from prior providers
Prescription Glasses or Contacts
- Wear your corrective lenses to the appointment
- Vision screening is part of the physical exam
- Bring your glasses case if you wear contacts
Payment

Interpreter Information
- You may bring a trusted interpreter if needed
- Notify us in advance if you require language assistance
- Children should not serve as interpreters
Vaccination Records — What We Accept
Accepted Formats
- CDC yellow vaccination card
- State immunization registry printout
- Foreign immunization booklet or record
- School vaccination records
- Military immunization records
- Written records from prior physicians
Foreign Records
We accept vaccination records from any country. Foreign-language records are acceptable — formal translation is not always required for vaccine documentation.
No Records? No Problem.
If you do not have vaccination records, we will review your history and administer any required vaccines at our office. We handle this situation regularly and will ensure your I-693 is fully USCIS-compliant.
For Children — Additional Items
Children being examined must be accompanied by a parent or legal guardian. In addition to the standard checklist, bring the following for minors:
- Original birth certificate (or certified copy)
- Parent or legal guardian government-issued photo ID
- School vaccination records
- Immunization records from pediatrician
- Legal guardianship documents (if applicable)
What NOT to Bring
- Unnecessary bulk medical records unrelated to your immigration case
- Large bags or luggage — our waiting area is compact
- Opened or unsealed prior I-693 envelopes
- Food or beverages (water is fine)
- Multiple family members who are not being examined (space is limited)
