First Visit: What to Expect

The first visit includes all components of the immigration medical exam:

• Check-in and paperwork review (15–20 minutes)
• Medical history review with the civil surgeon (15–20 minutes)
• Physical examination (15–20 minutes)
• Blood draw for laboratory testing (5–10 minutes)
• TB skin test (TST) placement — a small injection under the skin (5 minutes)
• Vaccination review and administration of any needed vaccines (15–30 minutes)
• Review of findings and next steps with the civil surgeon (10–15 minutes)

Why Is a Return Visit Required?

The tuberculin skin test (TST) requires a 48–72 hour waiting period before the result can be read. The injection is placed at the first visit, and you must return 48–72 hours later for the civil surgeon to measure the skin reaction.

This return visit is mandatory — the TST cannot be read at the first visit, and Form I-693 cannot be completed without the TST result. Plan your schedule to accommodate the return visit.

Return Visit: What to Expect

The return visit is brief — typically 15–30 minutes. The civil surgeon will:

• Measure the TST reaction at the injection site
• Record the result on Form I-693
• Review any laboratory results that have returned
• Discuss next steps if any findings require follow-up

When Will I Receive Form I-693?

Form I-693 is typically completed within 2–3 business days after the return visit, once all laboratory results have been received. The civil surgeon will seal Form I-693 in an envelope and provide it to you.

Can the IGRA Blood Test Replace the Return Visit?

Yes. The IGRA blood test (QuantiFERON-TB Gold or T-SPOT) is an alternative to the TST that does not require a return visit — the blood is drawn at the first visit and sent to the laboratory. However, IGRA results also take several days to return, so Form I-693 cannot be completed immediately regardless of which TB test is used. Ask the civil surgeon about IGRA testing if you prefer to avoid the return visit.
