The total timeline from scheduling to holding your sealed Form I-693 depends on two main factors: (1) how quickly lab results return, and (2) whether any follow-up is needed — such as additional vaccines or evaluation of abnormal lab results.

For most patients, the process takes 1–3 business days after the exam appointment. Patients with complete vaccination records and normal lab results are typically on the faster end of that range.

Step 1 — Scheduling Your Appointment

What happens: You contact the Civil Surgeon clinic, choose a date and time, and confirm your appointment.

What to do before your appointment: Gather your passport, vaccination records, and any relevant medical history documents. Having these ready before you arrive will make your appointment faster and may reduce your out-of-pocket costs.

Timing tip: Schedule your exam after you have filed Form I-485 and received your USCIS receipt notice, but before your USCIS interview. Getting the exam too early risks the form expiring before USCIS adjudicates your case — Form I-693 is valid for 2 years from the Civil Surgeon's signature date.

Step 2 — Your Exam Day

Here is what happens during your appointment, step by step:

Check-in and Paperwork

Verify identity, review health history questionnaire.

Physical Examination

20–30 min

Head-to-toe exam by the Civil Surgeon: vital signs, vision, hearing, and organ systems.

Vaccination Review

10–20 min

Civil Surgeon reviews your vaccination records against the USCIS schedule. Any missing vaccines are administered during this step.

Lab Specimen Collection

5–10 min

Blood draw for TB testing (QuantiFERON-TB Gold), syphilis (RPR), and gonorrhea (if applicable by age).

Total appointment time: 60–90 minutes for most patients. Patients with incomplete vaccination records may take longer.

Step 3 — Lab Results Processing

After your exam, your blood specimens are sent to a certified laboratory. The Civil Surgeon reviews all results before completing Form I-693.

If any results are abnormal, additional evaluation may be required — which can extend the timeline.

Step 4 — Form I-693 Completion

Once all lab results are in and reviewed, the Civil Surgeon completes, signs, and dates Form I-693. Here is what happens next:

The form is placed in an envelope, sealed, and signed across the seal by the Civil Surgeon.

You are notified that your form is ready for pickup or mailing.

You receive the sealed envelope — do not open it.

Important: The envelope must remain sealed. Never open it before submitting to USCIS. An opened envelope will be rejected.

Step 5 — Submitting to USCIS

There are two accepted ways to submit your sealed Form I-693 to USCIS:

Option A
Filing with Form I-485

Submit the sealed Form I-693 envelope together with your I-485 package. USCIS will open the envelope when they process your case.

Option B
Bringing to Your USCIS Interview

If you have already filed I-485 without the medical exam, bring the sealed envelope to your USCIS interview. The officer will open it at the interview.

Note: Ask your immigration attorney which submission method applies to your case. Both are accepted by USCIS.

Factors That Affect Your Timeline

Several variables can shorten or lengthen the time from your exam to your completed Form I-693 — and beyond.

Vaccination Records Availability

Having complete records means a faster exam. Missing records mean more vaccines are needed, which lengthens your appointment.

Lab Result Turnaround

Standard turnaround is 1–3 business days. Expedited processing options are available at our clinic.

Abnormal Lab Results

May require additional testing or specialist evaluation before Form I-693 can be completed, extending your timeline.

USCIS Processing Time

After submission, USCIS adjudication timelines vary widely by case type and service center — this is outside the clinic's control.
