What the Exam Includes for Children
Every child receives a complete immigration medical exam tailored to their age and vaccination history.
Age-Appropriate Physical Examination
A complete physical exam tailored to the child's age, including height, weight, vision, hearing, and developmental assessment.
Vaccination Review
Our Civil Surgeon reviews the child's immunization history and administers any missing age-appropriate vaccines required by USCIS.
TB Screening
Tuberculosis screening appropriate for the child's age. Children under 2 may receive an IGRA blood test instead of the skin test.
Form I-693 Completion
Our Civil Surgeon completes and signs Form I-693 for the child, sealed in the required USCIS envelope.
Age-Based Vaccine Requirements
USCIS requires age-appropriate vaccines based on the CDC immunization schedule. Here is a general overview by age group.
Infants & Toddlers (0–2 years)
- Hepatitis B (3-dose series)
- DTaP (Diphtheria, Tetanus, Pertussis)
- Hib (Haemophilus influenzae type b)
- PCV (Pneumococcal conjugate)
- IPV (Inactivated Polio)
- MMR (at 12–15 months)

School-Age Children (3–10 years)

- Tdap (booster)

What Parents Need to Bring
Having the right documents ready ensures a smooth exam visit and may reduce the number of vaccines your child needs.
- Child's vaccination records or immunization card (any language)
- Child's birth certificate
- Child's passport or government-issued ID
- Parent or legal guardian photo ID
- Any prior medical records relevant to the child's health history
- School immunization records if available
Tip: Bring All Vaccination Records
Even partial vaccination records can help. If your child received vaccines in another country, bring those records — we accept records in any language.
Missing records may mean additional vaccines are required. Titer blood tests can confirm immunity for some vaccines, but this adds time and cost. Bringing complete records is the best way to minimize what your child needs.
