What Is a Medical Vaccine Waiver?

A medical vaccine waiver — more precisely called a medical contraindication — is a documented medical reason why a specific vaccine cannot safely be administered to an applicant. When a valid contraindication exists, the civil surgeon documents it on Form I-693 and USCIS evaluates the exemption as part of your application.

Common Medical Contraindications

Recognized medical contraindications for immigration vaccine requirements include:

• Severe allergic reaction (anaphylaxis) to a vaccine component or prior dose
• Compromised immune system due to disease or immunosuppressive therapy
• Pregnancy (for live attenuated vaccines such as MMR and varicella)
• Age-based contraindications per ACIP guidelines
• Other documented medical conditions that preclude safe vaccination

How the Civil Surgeon Documents a Contraindication

The civil surgeon is responsible for evaluating and documenting medical contraindications on Form I-693. Documentation must include the specific vaccine, the nature of the contraindication, and supporting clinical information. A verbal history alone is generally not sufficient — medical records or documentation from a treating physician are typically required.

The civil surgeon will review your documentation and make a clinical determination. If a contraindication is valid, it will be noted on Form I-693 and submitted to USCIS for evaluation.

Religious and Moral Exemptions
