Current Form I-693 Validity Rules

Form I-693 validity is governed by USCIS policy, which has been updated over time. Under current USCIS guidance, Form I-693 is generally valid for two years from the date the civil surgeon signed the form. However, USCIS has also applied a four-year validity rule in certain circumstances, counting from the date of the medical examination rather than the civil surgeon's signature date.

Attorneys should verify the current validity rules directly with USCIS policy guidance at the time of the client's interview, as these rules have changed and may change again. The safest approach is to treat the two-year rule as the operative standard unless USCIS guidance explicitly provides otherwise.

How to Identify Expiration Risk Early

Expiration risk should be assessed at the time the medical exam is completed and monitored throughout the case. Attorneys should:

• Record the civil surgeon's signature date from the client's copy of the medical records (not from the sealed envelope)
• Calculate the two-year expiration date and calendar it as a case deadline
• Set a reminder 90 days before expiration to assess whether the interview is likely to occur before the form expires
• If the interview is not scheduled within 90 days of expiration, proactively discuss with the client whether a new exam should be scheduled

What to Do When the Form Has Already Expired

If the form has already expired and the interview is approaching or has been scheduled, the client must undergo a new immigration medical exam immediately. There is no mechanism to extend or revalidate an expired Form I-693.

The ideal timing for the immigration medical exam depends on the client's case type and expected processing timeline. For adjustment of status cases, the exam should be timed so that Form I-693 will still be valid at the interview date — with a comfortable buffer.

For cases with unpredictable processing times (such as cases pending at heavily backlogged field offices), attorneys face a genuine timing dilemma: schedule too early and the form may expire before the interview; schedule too late and the form may not be ready in time.

A practical approach for long-pending cases is to schedule the exam approximately 12 to 18 months before the anticipated interview date. This provides a buffer against processing delays while keeping the form within its validity window for most interview timelines.

When the Interview Is Rescheduled After the Exam

USCIS interview rescheduling is one of the most common causes of Form I-693 expiration. If the client's interview is rescheduled to a date that falls after the form's expiration, the client will need a new exam. Attorneys should monitor interview scheduling closely and be prepared to act quickly if a rescheduling creates an expiration problem.
