What Is NAAT Testing for Gonorrhea?

Nucleic Acid Amplification Testing (NAAT) is the gold-standard method for detecting gonorrhea (Neisseria gonorrhoeae). NAAT detects the genetic material of the bacteria and is significantly more sensitive and specific than older culture-based methods. It is the testing method required by USCIS for the immigration medical examination.

Who Is Required to Have Gonorrhea Testing?

Gonorrhea NAAT testing is required for all immigration medical exam applicants, regardless of age, gender, or immigration category. There are no age exemptions for gonorrhea testing under the current CDC Technical Instructions for Civil Surgeons.

How the Test Is Performed

Gonorrhea NAAT testing is typically performed using a urine sample or a swab specimen. The test is non-invasive and straightforward. All testing is performed on-site at our clinic on the day of your appointment — no separate lab visit is required.

Understanding Gonorrhea Test Results

A negative NAAT result indicates no gonorrhea infection was detected. This is the expected result for most applicants.

A positive NAAT result indicates active gonorrhea infection. Gonorrhea is a Class A communicable disease of public health significance and is a ground of inadmissibility under INA § 212(a)(1)(A)(i). Active untreated gonorrhea must be treated before Form I-693 can be completed and submitted to USCIS.

What Happens If You Test Positive

If your gonorrhea NAAT is positive, your Civil Surgeon will discuss treatment options with you. Gonorrhea is highly treatable with antibiotics. Current CDC-recommended treatment is a single intramuscular injection of ceftriaxone.

After treatment is completed and documented, your Civil Surgeon can finalize Form I-693. The treatment history is documented in the appropriate section of the form. A test of cure may be recommended depending on clinical circumstances.

How Results Are Documented on Form I-693

Gonorrhea testing results are documented in Part 4 of Form I-693 (Gonorrhea). Your Civil Surgeon records the test result and, if positive, the treatment provided and date of treatment. The completed form is sealed and submitted to USCIS.
