Vaccinations & Laboratory Testing
All CDC-required vaccines and USCIS-mandated lab tests completed at our Costa Mesa clinic — everything needed for your Form I-693.
Required Vaccinations
USCIS requires all applicants to receive age-appropriate vaccines on the CDC immunization schedule. Our Civil Surgeon reviews your vaccination history and administers any missing vaccines during your exam visit.
MMR (Measles, Mumps, Rubella)
Two doses required; titer testing accepted as proof of immunity
Varicella (Chickenpox)
Two doses or documented history of disease
Hepatitis B
Three-dose series; titer testing available
Tdap (Tetanus, Diphtheria, Pertussis)
One dose required; Td booster every 10 years
Influenza
Required annually during flu season (August–March)
COVID-19
Primary series required; boosters per CDC schedule
Meningococcal
Required for applicants ages 11–21
Pneumococcal
Required for applicants ages 65 and older
Laboratory Testing
USCIS requires specific laboratory tests as part of the Form I-693 immigration medical exam. All required tests are ordered and coordinated by our Civil Surgeon.
Mantoux Tuberculin Skin Test (TST)
Standard TB screening placed at the exam visit; read 48–72 hours later.
IGRA Blood Test (QuantiFERON)
Interferon-gamma release assay — a single-visit alternative to the skin test.
Syphilis Serology (RPR/VDRL)
Required for all applicants age 15 and older.
Gonorrhea Testing
Required for all applicants age 15 and older.
Urinalysis
Routine urine analysis performed on-site.
Vaccine Documentation
Bringing your vaccination records can reduce the number of vaccines you need and lower your overall cost. Here is what to bring:
- Original vaccination records or immunization card
- Records from your home country (any language accepted)
- Prior titer test results showing immunity
- School or military immunization records
- Any prior Form I-693 or vaccination supplement
Foreign Records & Translations
We accept vaccination records from any country. Records do not need to be in English — our staff is experienced in reviewing international immunization documentation. If a certified translation is required, we can advise you on next steps.
No Records Available?
If you have no vaccination records, titer blood tests can confirm immunity for certain vaccines (MMR, Varicella, Hepatitis B). If titers are negative, the vaccines will be administered. Our team will guide you through the most cost-effective approach.
Vaccine Waivers
In limited circumstances, USCIS allows vaccine waivers for applicants who cannot receive certain vaccines. Two types of waivers are available:
Medical Contraindication
If a vaccine is medically contraindicated due to allergy, immune deficiency, or other documented medical condition, our Civil Surgeon can complete a waiver with supporting documentation from your physician.
Religious Belief
Applicants with sincere religious beliefs opposing vaccination may request a religious waiver. This requires a written statement and is subject to USCIS review. Our Civil Surgeon can document the waiver request on Form I-693.
